Assessment Center
In Partnership with Penn Medicine Lancaster General Health
Located on the second floor of the Commons Building, the Assessment Center will aim to advance community wellness through education and the prevention of neurodegenerative disease. An interdisciplinary team will provide comprehensive care for Residents of Willow Valley Communities, and will be open to serve the broader region.
Areas of Focus
- Assessment – offering convenient access, enabling earlier identification and connections to services, sooner
- Treatment – specializing in individualized care, the latest in breakthroughs, and the best practices for memory care
- Prevention – delivering the tools needed to promote a brain-healthy lifestyle, warding off cognitive decline
- Education – enhancing public awareness, eliminating stigma, improving wellbeing, and reducing caregiver stress
- Support – providing a hub for everyone impacted by memory loss, elevating quality of life for an entire community
- Research – advancing knowledge of risk factors, available treatments, preventive measures, and neurological care
- Trials – making trials more widely available, accessible, and simplified for those who desire to participate
An initial evaluation to establish a baseline coupled with routine follow-up assessments with the practitioner will be essential to developing a personalized approach for each patient. These ongoing check-ins provide the opportunity to re-evaluate the individual maintenance or treatment plan, while also potentially preventing hospitalizations, falls, and unnecessary caregiver stress.
